HRT Monitoring and Follow-Up: What Good Care Looks Like
Starting hormone therapy is the easy part. Good outcomes come from disciplined follow-up over months and years.
Standard schedule
Follow-up at 6–8 weeks (symptom review, tolerability), 3 months (repeat targeted labs, dose adjustment), 6 months, then annually with focused clinical review, blood pressure, weight, breast and pelvic exam when due, and updated labs.
What we monitor
Women on estrogen ± progesterone: TSH, lipids, liver enzymes, and clinical breast exam schedule. Men on testosterone: total testosterone, hematocrit, PSA, lipids. Any patient: blood pressure, weight, and quality-of-life symptom scoring.
When to adjust
Persistent symptoms at 12 weeks despite adherence, unfavourable lab drift (rising hematocrit, PSA change, abnormal lipids), new side effects, or life events (surgery, new medications, cancer diagnosis). Adjustments are collaborative — never abruptly stopped without a plan.
