Managing GLP-1 Side Effects: A Practical Playbook
Most GLP-1 side effects are predictable and manageable. Here is the playbook we use with our patients.
Nausea
Peaks in the first week after each dose increase and typically resolves within days. Prevention: smaller meals, low-fat and low-fried foods during titration, no eating past fullness. If persistent, we hold the current dose an extra 2–4 weeks before escalating.
Constipation
Very common because gastric emptying slows. Water intake > 2 L/day, 25–35 g fibre, magnesium citrate 200–400 mg at bedtime, and light daily activity resolve most cases. Persistent cases benefit from PEG 3350.
Reflux and early satiety
Sit upright for 60 minutes after meals, avoid trigger foods (fatty, spicy, mint, coffee) during titration, and consider a short course of a PPI if symptomatic. Extreme early satiety warrants dose adjustment.
Fatigue and dizziness
Usually reflect inadequate intake. Baseline is 1.2 g/kg protein, 20–25 g carbohydrate per meal, and 2 L water per day. Blood pressure medications may need downtitration as weight comes down.
